Monroe County Clerk - Auto License Bureau

Our mobile DMV unit is able to provide many basic DMV services, including processing requests for non-driver identification cards.  We can also provide forms, information, and "rain checks" for those who want to get a non-driver ID but don't have all the required forms of ID with them on the day of the event.  booth 1A

Social Security Administration

The Social Security Administration will be available to take applications for replacement Social Security cards, set up appointments to file for benefits and answer general questions you may have about Social Security benefits. booth 2

We assist with housing issues, public benefits, consumer issues, health insurance issues, and employment and housing discrimination. This encompasses sanctions, denials and discontinuances of public benefits, including Temporary Assistance and SNAP/Food Stamps, denial of applications for housing because of credit and criminal history, and other problems. booth 75

YourCare Health Plan

A not-for-profit, patient-centered organization providing access to free or low-cost health insurance options such as Medicaid Managed Care, Child Health Plus, and the Essential Plan. Facilitated enrollers will provide information and assist with enrollment. Booth 35

Monroe Plan for Medical Care

The parent company of YourCare Health Plan — dedicated to providing outstanding Health Home Care Management Services.  Care Managers will help those eligible for Medicaid Managed Care, Child Health Plus, and the Essential Plan navigate through their medical, mental and social service health needs. booth 35

Trillium Health

We will provide blood pressure screenings, information on PrEp (Pre-Exposure Prophylaxis) for HIV, care management (housing, transportation, food cupboard information, health homes care management services), health insurance navigation and enrollment, and Opioid overdose prevention training/naloxone distribution will be provided in booth 56. Rapid HIV and HCV testing, and counseling in the private rooms.

Anthony Jordan Health Center

Patient registration, dental screening, MedicAid enrollment. booth 9

Eastman Community Dentistry

Highland Family Planning

Free and confidential birth control services as well as STD testing and treatment. We accept all insurances but also help people who don't have insurance. booth 61

Health Reach

We will provide routine dental services in our large mobile dental unit parked in the front of the Blue Cross Arena.  Inside the private room near the coat room, our physician and support staff will be providing primary care medical services.  This will include treatment of acute illnesses as well as management of chronic diseases and health maintenance visits.  Individuals can sign up to receive ongoing care from our doctors at local shelters, on our mobile unit, or at our clinic on 819 West Main. Our facilitated enroller will help individuals sign up for health insurance.  Our case managers and medical assistants will help people with a variety of needs /concerns and also screen for high blood pressure.  Hablamos Español! booth 60

UR Street Medicine

We will be providing blood pressure checks, blood glucose checks, hygiene supplies, and other basic medical assistance. We will also be enrolling homeless individuals with recent Emergency Department stays into a new program that provides people with a team of students who can help coordinate medical and social care to get individuals to a more stable medical situation. booth 57

Cancer Services Program of Monroe County/ UR Center for Community Health

Breast, cervical and colorectal cancer screening information; healthy living recommendations, enrollment for free cancer screenings available for eligible adult men and women.  booth 59

We provide medicaid long-term care services and intensive case management to people with chronic medical and mental health issues. Personalized home care agency providing service coordination via TBI, NHTD and Health Home programs, HHA/PCA service, HCBS, and CDPAP. Coordinated Care Services Inc.

Enrollment Services for persons needing assistance with Medicaid programs. Medicaid, Medicaid Excess resource/ income program, Medicare Savings Program and the Medicaid Buy In for Working People with Disabilities. MCOMH is committed to improving the health and quality of life of county residents who are affected by mental illness, developmental disabilities, alcohol and other drugs. Recovery Houses of Rochester, Inc.

We are looking to Peer to Peer Supportive Housing and Mentoring to Recovering Alcoholics and Addicts. Male and females over 18 yrs. of age. Personal Development Group every Thursday 6pm at 285 Ormond Street. booth 33

Our program offers pregnancy education, mental health counseling, care management and job training/education services. National Council of Jewish Women

We will provide period kits--menstrual supplies of pads, tampons and panty  liners. We will also provide bras in our private fitting room, with help measuring and fitting. booth 65

Women's Initiative Supporting Health

Willow Domestic Violence Center

Disabled American Veterans Chapter 15

Providing free, professional assistance to veterans and their families in obtaining benefits and services earned through military service and provided by the Department of Veterans Affairs (VA) and other agencies of government. booth 30

Rochester Vet Center

We provide counseling to war zone veterans and their families, to veterans who have had sexual trauma while in the military, and to families of veterans killed in the line of duty. We educate the community about PTSD and services for veterans. We link veterans with medical services, housing, benefit, education and employment services. booth 39

U.S. Department of Veterans Affairs

Open Door Mission

The Samaritan House Crisis Housing Center at the Open Door Mission provides safe, healthy emergency overnight housing to homeless men, including access to bathroom and shower facilities and a hot meal. We help with some case management. booth 50

Person Centered Housing Options (PCHO)
PCHO and MC Collaborative with the Library Resource Outreach Center will provide navigation and support to all participants and volunteers at the event in finding resources, accessing resources not available at the event, and linking people to the appropriate services to help make their day productive and enjoyable. We will also be the station that certifies that someone can get a birth certificate through a process with DHS. booth 85

211/ LIFE LINE Goodwill of the Finger Lakes

Our 24/7 hotline provides information and referral for human services, along with crisis counseling and suicide intervention. We are currently participating in the Emergency Housing Shelter Screenings in partnership with Coordinated Access. We can provide referrals to services including housing, food, shelter, mental health, substance abuse, employment, education, health related services, criminal justice and legal resources, and other family and community resources booth 66

Life Decisions Program

Our volunteers serve as agents for signing the NYS Agent Appointment form.  We fill out the form which states a person's wishes at the time of their death and lists anyone who should be contacted in the event of their death.  The paperwork is then filed with the county. booth 68
