Monroe County Department of Human Services

DHS will be able to answer questions about applying for benefits such as cash assistance, SNAP, Medicaid, emergency assistance, etc. and inquiries on active or pending applications for assistance. Booth 5

ID

Monroe County Clerk - Auto License Bureau

Our mobile DMV unit is able to provide many basic DMV services, including processing requests for non-driver identification cards.  We can also provide forms, information, and "rain checks" for those who want to get a non-driver ID but don't have all the required forms of ID with them on the day of the event.  To complete an application, individuals need to have either had an ID issued in the past (after 2003) or have their birth certificate and social security card in hand. Booth 7

Social Security Administration

We will be available to set up appointments and take applications for replacement Social Security cards. We can also answer quick, basic questions people may have about their benefits. Booth 4

Monroe County Health Department – Vital Records

If you were born in Monroe County, you can get a birth certificate on the spot. To be eligible for a free birth certificate, you will need a voucher, which may have been given to you by a shelter in the days before PHCR.  If you are homeless and don't have a voucher, please go to the DHS booth to check eligibility. MEDICAL

YourCare Health Plan

A not-for-profit organization, providing free or low-cost health insurance plans, such as Medicaid Managed Care, Child Health Plus, and the Essential Plan, to residents in seven counties across the Greater Buffalo & Rochester regions. Facilitated Enrollers will share information and assist with enrollment on-site. Booth 2

Monroe Plan for Medical Care

The parent company of YourCare Health Plan — dedicated to providing outstanding Health Home Care Management Services.  Care Managers will help those eligible for Medicaid Managed Care, Child Health Plus, and the Essential Plan navigate through their medical, mental, and social service health needs. Booth 3

Trillium Health

Our team will provide patient registration, HIV/HEP C testing, information on PrEp (Pre-Exposure Prophylaxis) for HIV, care management services (housing, transportation, food cupboard information, health homes care management services), and insurance navigators. LL3

Anthony Jordan Health Center

Activities include dental screening, patient registration, HIV/Hep C Testing, and facilitated enrollment. Booth 33

Eastman Community Dentistry

Dental screenings, toothbrush and toothpaste, dental education. Booth 31

Health Reach

Health Reach will provide routine dental services on our Mobile Medical Unit parked in front. Inside, our medical team will have two private rooms for our physicians to treat attendees with acute illnesses, provide health screenings, and management of chronic medical conditions. Our intensive case managers and therapists will be available to consult with attendees and schedule future appointments.  Individuals can sign up with Health Reach to receive ongoing care from our providers at our clinic, on our Mobile Medical Unit, or at local homeless shelters that we partner with. We provide services to patients regardless of ability to pay. We will have an insurance enroller on-site to assist patients. Booth LL1

Rochester Regional Health Mobile Mammography

Mammograms will be given in the mobile van at curbside, but patients must check in at  Booth LL2

Saratoga Rx LLC

Medication packaging, compounding, dispose my meds, immunization, free delivery, DME products. Will provide flu vaccine, shingle vaccine, and pneumonia vaccine. Booth 35

UR Street Medicine

We provide basic medical care and medical supplies to people in need. Booth 37

Cancer Services Program of Monroe County/UR Center for Community Health

Cancer prevention and wellness.  Information on free breast, cervical and colorectal cancer screening for eligible adult women 40+ years and men 50+ years old. Booth 38

Venture Forthe Inc.

We provide medicaid long-term care services and intensive case management to people with chronic medical and mental health issues. Booth 39

Coordinated Care Services Inc.

Navigators to provide health insurance enrollment through the NYS of Health and facilitated enrollers to provide application assistance for Medicaid programs for the aged, blind and disabled.   Booth 29

Health Home of Upstate New York

St. Joseph's Neighborhood Center, Inc.

The Neighborhood Center provides comprehensive health care, dental care, mental health counseling and social work services to individuals who lack access to health insurance. Booth 42

Epilepsy-Pralid, Inc.

We are a home care agency who focus on a variety of services for members, such as developmental disabilities, case management (Health Homes/service coordination) PCA, TBI, residential for clients with disabilities and more. Booth 43

Glasbergen Insurance Services

The Glasbergen Insurance Services team offers Medicare benefits and program assistance to low income individuals and seniors aged 65 and over. We also offer life  insurance and dental insurance products. Booth 44

Lifetime Financial Group, LLC

Lifetime Financial Group is an independent agency offering all types of insurance. Specializing in healthcare both on the New York State Health Exchange and Medicare. We offer help with enrollment in health-related programs that will save you money! Booth 45

LEGAL

LawNY

We assist with housing issues, public benefits, consumer issues, health insurance issues, and employment and housing discrimination. This encompasses sanctions, denials and discontinuances of public benefits, including Temporary Assistance and SNAP/Food Stamps, denial of applications for housing because of credit and criminal history, and other problems. Booth 9

Empire Justice Center

Advice and/or intake on civil legal services such as SSI/SSD, foreclosure, education law, LGBT, workers’ rights. Booth 11

Disability Rights New York

Disability Rights New York is the Protection & Advocacy System and Client Assistance Program for persons with disabilities in New York. New York residents with disabilities can contact DRNY for assistance with problems directly related to their disabilities. DRNY investigates complaints and provides direct assistance to individuals with disability-related issues under the terms of its federal grants and its annual goals and priorities. Booth 13

Volunteer Legal Services Project

VLSP provides free legal help to low-income residents of Monroe County who are facing non-criminal legal problems. We handle Family Law matters (custody, visitation, divorce for individuals experiencing domestic violence, child support), Consumer Law matters (debt collection, bankruptcy, unemployment insurance, tax issues), and Health Law matters (legal services for HIV positive individuals, wills and advance directives for seriously ill clients). MENTAL HEALTH

Monroe County Department of Mental Health

Provide case management referrals, RED referrals, SPOA information, information for outpatient mental health and substance abuse services. Booth 55

Recovery Houses of Rochester, Inc.

We provide peer-to-peer supportive housing and mentoring to recovering alcoholics and addicts, both male and female, over 18 years of age. We will do screenings for potential applicants and provide information about what we offer. Booth 49

Evelyn Brandon Health Center

Information and access to connect with chemical dependency treatment  and mental health treatment within the Greater Rochester, NY area. Booth LL4

Alzheimer's Association

Information about Alzheimer's disease and related dementias. Alzheimer's Association offers free programs and services to support those living with dementia and their caregivers/family/friends. Booth 51

Huther Doyle

Huther Doyle provides walk-in evaluations for individuals struggling with substance use. Group therapy is offered to assist individuals on their road to recovery. We deliver person-centered therapy and a variety of medication-assisted treatment options with our newly opened MAT clinic. Huther Doyle is teaming up with community providers to offer a new, well-rounded approach to living a recovery lifestyle. Booth 52

ROCovery Fitness

ROCovery Fitness provides a multitude of free services to ANYONE with 48 hours of continuous sobriety which include: strength & conditioning groups, meditation, weight lifting, hiking, park workouts, yoga, kayaking, bike rides, camping, sober support meetings, social events and MUCH more!!! URMC- HEAL collaborative

HEAL is the service provided to patients experiencing issues related to violence or trauma. HEAL provides mental health, legal and social services – in the same place, at the same time – to individuals who are 18 years of age and older to help ensure their safety and well-being. Booth 54

Spiritus Christi Prison Outreach

Information and access to connecting with chemical dependency treatment within the Greater Rochester, NY area. Transitional housing at Nielsen House (men) and Jennifer House (women). Booth 55

Catholic Charities Community Service

We offer care management service to both children and adults with chronic medical conditions, mental health diagnosis and traumatic brain injuries. Booth 47

SERVICES TO WOMEN

National Council of Jewish Women/I Support the Girls

We will provide menstrual hygiene kits (pads, tampons and panty liners). We will also provide bras in our private fitting room, with help measuring and fitting. Booth 56

Ask a GYN

Have questions about “female problems”? Ask a friendly gynecologist. Booth 57

Highland Family Planning

We provide low-cost and confidential birth control services.  We accept all insurance plans but also help people who are underinsured or insured get access to services. Booth 58

Women's Initiative Supporting Health

Medical care, mental health/substance use disorder referrals, and trauma informed peer navigation assistance for women re-entering from incarceration and with other justice involvement. We also have paid research opportunities. Booth 61

Willow Domestic Violence Center

Willow provides residential and non-residential services for survivors of domestic violence, including a 24/7 Hotline, emergency shelter, counseling, court advocacy and prevention education services. Advocates from our Mobile Program will be providing safety planning and brief consultations for those who would like to talk. Booth 59

Healthy Baby Network

We work with mothers and families to provide access to care, job training, and housing security to ensure a healthy, stress free pregnancy. Booth 60

VETERAN SERVICES

NYServes Upstate

We guide serving veterans, service members and their families to the most appropriate services and resources available in their area. Care coordinators work via phone and web to intake, assess and connect veterans to providers who can help them. Booth 17

Disabled American Veterans Chapter 15

Providing free, professional assistance to veterans and their families in obtaining benefits and services earned through military service and provided by the Department of Veterans Affairs (VA) and other agencies of government. Booth 18

Rochester VA Outpatient Clinic Veteran Service Center

We help veterans enroll or register for healthcare with the VA and talk to them about accessing benefits they may be entitled to. Booth 19

Veterans Outreach Center, Inc.

VOC is a one-stop service center for veterans, guard, reservists, active duty and immediate family members.  We offer emergency, transitional and permanent housing; employment and training; wellness and creative arts therapy; financial and legal counseling; business clothing; education benefit counseling, temporary financial assistance, peer to peer mentoring, residential & substance abuse services and so much more. Canandaigua VAMC and Rochester Outpatient Clinic

Suicide prevention information, including information about the Veterans Crisis Line for veterans, families, and friends of veterans in crisis. Info about Veterans’ services and how to register for benefits. Booth 21

VA

Provide housing and/or housing assistance for Veterans who are homeless or at risk of becoming homeless. Booth 22

NYS Division of Veterans Affairs

Overall VA and NYS veterans’ benefits. Compensation and NSCP claims navigation, military discharged upgrade, etc. Booth 23

Warrior Salute Veteran Services

WSVS helps veterans with post-traumatic stress disorder, traumatic brain injury and military sexual trauma regain their lives by providing quality clinical and rehabilitative therapies, transitional housing and case management. Staff work one-on-one with veterans, and their families, to help identify their goals, determine the personalized support needed and implement the services and training necessary to foster growth and development as they transition back into the civilian community. Warrior Salute Veteran Services welcomes both male and female veterans as participants.  Outpatient services include individual mental health counseling, group therapy, art therapy, mindfulness, cognitive behavior therapy, case management services, family support. The Housing Council at PathStone, Inc.

Provides landlord education, apartment listings, fair housing education, tenant rights and responsibilities, foreclosure prevention counseling and first-time homebuyer education. Booth 71

Catholic Family Center

Safe emergency shelter for men, women, women with children and families; access to health care & social services; onsite case management with additional supportive services to help clients move to safe permanent housing. Booth 72

Open Door Mission

ODM currently has a men’s homeless shelter, addiction recovery program, and woman's and children's transitional shelter. We also have a community dinner, men's, women's and children's clothing ministry. Booth 73

Volunteers Of America

Housing for homeless men. We’ll interview potential tenants and schedule tours. Booth 74

YWCA of Rochester

We provide housing programs such as supportive living for the CD population, shelter for the homeless, SRO, and apartments in the community. We have a young adult and young mothers’ division. Booth 75

Salvation Army

We provide emergency housing for homeless men, women & children at Booth Haven and Hope House. We can help with immediate assistance with any emergency housing needs. Booth 76

House of Mercy

Open 24/7/365 House of Mercy provides compassionate emergency shelter to adults; day drop-in center; soup kitchen; various support groups; food pantry; clothing room; computers; individual and system-wide advocacy; and support services. Booth 77

CHILDREN/YOUTH

The Center for Youth

We serve youth aged 12 - 24 with counseling, housing support, food, clothing and job readiness training. Our programs include runaway and homeless youth services as well as school base prevention counseling. Booth 62

The Salvation Army

A 30-day emergency shelter for homeless youth ages 16-20. Booth 63

Rochester City School District

We protect students’ educational rights when they are experiencing housing instability. We assist with registration/school placement, transportation, food services, clothing, school supplies, case management, bus passes. Booth 64

Person Centered Housing Options (PCHO)

PCHO will provide navigation and support to all participants and volunteers at the event in finding resources, accessing resources not available at the event, and linking people to the appropriate services to help make their day productive and enjoyable. Booth 6

MC Collaborative

We provide complex Care Management for individuals facing significant mental health and/or addiction issues. We also provide numerous other support services in the community for people facing issues of poverty, homelessness, etc. Booth 25

211/ LIFE LINE Goodwill of the Finger Lakes

Our 24/7 hotline provides information and referral for human services, along with crisis counseling and suicide intervention. We can provide referrals to services including housing, food, shelter, mental health, substance abuse, employment, education, health related services, criminal justice and legal resources, and other family and community resources. Lifespan of Greater Rochester Inc

Helping older adults over the age of 60 age in place with information, resources, and application assistance. Booth 79

Medical Solutions, Inc.

We provide free health coaching for Medicaid recipients and the uninsured. If there is any aspect of our clients’ life that is causing stress, disease or illness, we will work with them to help figure out solutions. We also offer supplemental medical staffing to local hospitals and nursing facilities and are a part of the CDPAS program.  Booth 80

FOOD/NUTRITION

Asbury Dining and Caring Center

We provide 6 days of meals, showers, laundry, hair care, hygiene bags, bike repairs, medical clinic and clothing storehouse. Booth 65

Cornell Cooperative Extension

We will provide nutrition education. Booth 66

Cameron Community Ministries

We provide lunch meals Mondays thru Saturdays and dinner on Sundays. In addition, Cameron has food pantry (by appointment) and a mobile food drive every third Friday of the month. There is an afterschool youth program and summer youth program. Cameron also has a clothing house ($1 per person in household) open Tuesdays thru Thursdays from 9:30am - 12:30pm. Booth 67

St Joseph's House of Hospitality

We are a soup kitchen, serving meals 7 days a week, and an emergency men's shelter. We offer free laundry, clothing and showers. We also run Rochester Free Store, where we give away clothing and household items. We are a Catholic Worker House, we also have a “housing first” apartment house for the chronically homeless, called Dorothy Day House. Booth 68

ACCES-VR

ACCES-VR provides vocational rehabilitation services to individuals that have a documented disability that presents a barrier to employment.  Booth 86

Pathstone Employment Focused Services

Employment Focused Services provides job readiness training, free vocational training, and employment services to individuals who are currently unemployed and on probation. Booth 89

Rochester Construction Training Center

We have a 12-week training program that covers construction skills leading to well-paying jobs.  No academic prerequisites and a criminal record is not an obstacle. Booth 87

Monroe County Workforce Development/Rochester Works

We will provide information about a number of different city and county initiatives to help people get training and placement in industries with labor shortages. Booth 88

Veterans Integration to Academic Leadership

The mission of the VITAL program is to help veterans achieve their academic goals. VITAL staff can assist with advocacy for Student Veterans.  VITAL staff consists of mental health professionals who assist the Veteran and their university/college by providing services and resources. Booth 93

Rochester Rehabilitation Center—Employment Division

Employment, job readiness, benefits assistance, jobs, mental health services and vocational training linkage. Booth 90

Center for Employment Opportunities

We work with individuals who have been recently released from prison with temporary employment while assisting them with finding and maintaining permanent employment. Booth 94

Turning Points Resource Center
